Frequently asked questions

Is this legally binding in my state?
It is a professionally structured template, but contract and consumer rules vary by state. Have a licensed attorney in your state review your finished version, especially the liability, refund and dispute clauses.

Can I use it if I bill Medicaid or insurance?
Yes, but your state’s Medicaid program or the insurer may require specific wording. The agreement marks the spot to paste those required terms.

What if I don’t have a backup doula?
The agreement includes a checkbox option that states clearly that you have no backup arrangement and what happens if you can’t attend. DONA International’s standards ask doulas to disclose this in their contract.

Can I change the prices, weeks and policies?
Yes. Everything is editable: retainer, balance due week, on-call weeks (37–42 by default), travel radius, refund amounts, shift minimums and more.

Does it include a contract for end-of-life doulas?
No. The End-of-Life Doula Business Kit has a separate agreement written for that work.

Do I get the forms too (intake, birth plan, invoices)?
Those are in the Doula Business Kit, which includes these contracts.
