| Visit cancellation [24] hours; payment within [__] days | Client-facing terms | Put the same terms in your service agreement |
| Personal care sections | Delete, or keep if licensed | Keep them only if you also hold a home health agency license |

For the rules behind each policy, read Florida homemaker and companion agency policies and procedures.
Adopt it
- Finish every bracket, delete personal care content if you’re registration-only, and update the table of contents.
- Have it reviewed.
- Have your governing body sign the adoption page, train staff at orientation and keep signed acknowledgments (Form F-01).

FAQ
Is there an AHCA-approved homemaker and companion template?
We didn’t find any process for AHCA to approve a policy template. Be careful with listings that claim state approval. Look for citations you can check instead.
Does the template include personal care policies?
Yes, but they’re marked not permitted for registrants. Delete them unless you also hold a home health agency license.
Is it editable?
Yes. You get an editable Word document plus a PDF, and it also opens in Google Docs.
Why does Florida leave so many numbers to me?
Section 400.509 and Rule 59A-8.025 focus on registration, screening, advertising and records. Training, supervision and retention are left to the agency, its payers and its insurer.
